What You’ll Do

Provide skilled nursing care in patients’ homes

Follow clear care plans and clinical protocols

Monitor vitals, medications, and medical equipment

Document visits in Kantime EMR (modern system)

Coordinate care with families and clinical team

Support safe transitions and continuity of care
Must-Haves
-
Active Texas RN license
-
Available for 36–40 hrs/week

Able to complete home visits in assigned counties

Comfortable working in home health settings
Nice-to-Have

Kantime EMR or EVV experience

Prior home health experience

Bilingual (Spanish)
