What You’ll Do

• Oversee patient care plans and ensure quality care for patients in their homes.
• Conduct patient assessments and develop care plans tailored to individual needs.
• Collaborate with doctors, therapists, and other healthcare professionals to coordinate care.
• Perform routine visits to monitor patient progress and adjust care plans as needed.
• Educate patients and families on care plans, medications, and health management.
• Document patient care accurately and in a timely manner.

What You’ll Need

• Active RN license.
• Home health experience preferred, but new grads will be considered (limited availability).
• Strong organizational and communication skills.
• Ability to work independently while managing multiple priorities.
