Role Snapshot

  • Hospice RN Case Manager • Naperville & suburban Chicago • In-home visits
  • Pay: $45-$48/hr based on experience
  • Flexible schedule with shared evenings/weekends/holidays
  • You’ll guide patients and families through end-of-life care with skill, calm, and compassion.

What you’ll do

  • Manage your own patient caseload

  • Build and adjust care plans that fit each person’s needs

  • Teach families how to manage symptoms and what to expect

  • Communicate with physicians and the IDT team

  • Chart visits bedside using a tablet (NetSmart EMR)

  • Support patients and families during the final stage of life

  • Take part in care reviews and limited on-call rotation

Must-haves

  • Active Illinois RN license (unrestricted)

  • 1+ year RN experience

  • Reliable car + valid driver’s license

  • Comfortable working independently in homes

  • EMR experience (any system)

Nice to have

  • Hospice or palliative care background

  • Familiarity with NetSmart EMR
