What You’ll Do
-
Provide direct, in-home nursing care
-
Conduct health and psychosocial assessments
-
Educate patients and families on conditions and care plans
-
Collaborate with PTs, LPNs, OTs, and Social Workers
-
Complete accurate OASIS documentation (admission, transfer, recertification, etc.)
-
Attend case conferences and team meetings
What You’ll Need

Active RN license in Virginia
-
Three or more years of nursing experience (home health preferred)
-
Valid driver’s license and reliable transportation
-
CPR certification
-
Ability to pass background checks and provide references
