Here’s What You’ll Actually Do:

Run your own patient panel

Do in-depth assessments and manage chronic conditions

Close care gaps (HEDIS and STAR)

Lead care planning with a full clinical team

Guide patients and families so they feel confident in their care
You’ll Need:

Unrestricted APN license and board certification

Eligible to treat Medicare patients

6+ months in a skilled nursing or long-term care setting

Comfortable with digital tools and team communication
