What You’ll Do:
- Provide exceptional nursing care to patients in their homes
- Collaborate with healthcare teams to create and implement care plans
- Communicate effectively with patients, families, and team members
- Demonstrate empathy and compassion in all patient interactions
- Travel throughout the county to complete hospice visits
What You Need:
- Valid RN license in California
- At least one year of experience in hospice or home healthcare
- Certification in hospice and palliative care (CHPN, CHPPN, CHPCA, etc.) is a plus
- Proficiency in English; bilingual in Spanish or another relevant language is a plus
- Strong communication skills and ability to work collaboratively
- Willingness to work flexible hours, including evenings and weekends
- Ability to drive to patient homes throughout the county
