What You’ll Do:
- Provide direct nursing care in home settings
- Perform comprehensive health and psychosocial evaluations
- Monitor and educate patients on their conditions
- Coordinate with Physical Therapists, LPNs, OTs, and Social Workers
- Complete OASIS documentation (admission, transfer, recertification, etc.)
- Participate in case conferences and staff meetings
What You’ll Need:
- RN license in good standing (VA)
- Three or more years of RN experience (home health preferred)
- Reliable transportation and valid driver’s license
- CPR certification
- Ability to pass background checks and provide references
