Join a trusted hospice network growing fast across Texas. Our client needs a field-based RN Case Manager to serve patients in and around Victoria, TX. You’ll have autonomy in your day, real support from leadership, and the time to give care the right way.

What You’ll Do

• Manage a field caseload delivering hands-on hospice care.

• Coordinate with patients, families, and the full care team.

• Keep documentation clean, accurate, and compliant.

• Rotate in on-call coverage (extra pay).

• Handle occasional after-hours visits (paid per visit).

• Bring steady, compassionate care to patients and families in need.

Must-Haves

• Active RN license in Texas.

• Strong clinical judgment and fieldwork comfort.

• Reliable, compassionate, patient-first mindset.

• Flexibility for on-call and after-hours visits.

Nice to Have

• Prior hospice experience (training provided for motivated RNs).

Pay & Perks

• $75,000–$80,000 base salary (DOE).
