What you’ll actually do:

Own your patient panel and build long-term relationships

Lead care plans and do in-depth assessments

What you’ll need:

Active NP license and national certification

Medicare eligibility
-
6+ months experience in SNF or LTC settings

Comfortable with digital tools and remote team coordination
