Role Snapshot
- Role: Licensed Practical Nurse (Home Health)
- Location: Virginia Beach & Norfolk, VA • Onsite (home visits, local travel only)
- Pay: $48 per visit (~$70,000/year avg.)
- Schedule: Full-time, Monday–Friday, 8am–5pm • 5–6 visits/day
- Work with a stable, Medicare-certified home health team serving Hampton Roads. Clear routes, consistent workload, and full benefits.
What You’ll Do

Conduct home health visits and basic assessments

Deliver wound care, med administration, and skilled nursing

Support start-of-care and follow-up visits (no OASIS lead)

Coordinate with RN case managers and therapists

Complete visit notes within 72 hours

Communicate changes in patient condition promptly
Must-Haves
-
Active LPN license in Virginia (good standing)

6+ months of home health experience

Current CPR certification

Valid driver’s license and reliable insured vehicle

Must pass OIG, state, and national background checks
