We're seeking a nurse practitioner that will be based in Syracuse, but willing and able to travel throughout New York a couple weeks per month completing medication reviews, HEDIS and Stars quality gap closures, and promoting membership growth at different facilities.
This role requires flexibility to fill in across facilities within a state market, addressing vacancies and absences. Travel schedules vary and are collaboratively planned. Here's your mission:
- Provide exemplary primary care to a dedicated panel of members, aligning with their specific healthcare goals. This means treating in the most suitable setting, advocating for the 'treating in place' model, and navigating the complexities of advanced care planning.
- Conduct thorough assessments, craft personalized care plans, and ensure seamless delivery of the Model of Care activities. Your work will directly influence the quality of life of your patients by offering tailored, compassionate care.
- Play a pivotal role in clinical operations functions such as prior authorization, medication reviews, and the closure of HEDIS and STARs quality gaps, all while fostering membership growth.
- Be the cornerstone of communication and coordination, liaising between members, their families, and care teams. Educate and inform all stakeholders of care needs, treatment changes, and the overarching goals of care.
Must-haves for your toolkit:
- State unrestricted licensure as an Advanced Practice Nurse complemented by national board certification. Eligibility to participate in Medicare is a must.
- Medicare registered in the state of New York
- Active DEA license
- Active NPI
- A proven track record with at least 6 months of direct experience as an APP, preferably with exposure to geriatric, hospice, or palliative care populations. Experience in the nursing home environment is highly desired.
- Proficiency in technology use, alongside outstanding communication abilities to effectively connect with patients, families, and healthcare teams.
- Demonstrated clinical prowess in assessment, diagnosis, and treatment, especially tailored to the needs of the chronically ill.
