Here’s What You’ll Actually Do:

Manage your own panel of Medicare patients

Handle full assessments, treat chronic conditions

Close quality gaps (HEDIS, STAR, etc.)

Lead care planning alongside RNs, social workers, and health coaches

Be the point of trust for patients and their families
You’ll Need:

Active, unrestricted APN license and board certification

Eligible to treat Medicare patients
-
6+ months in skilled nursing or long-term care

Comfortable with EHRs and digital care tools
