As a Clinical Manager (RN), you’ll guide a dedicated home health team delivering high-quality, compassionate care to patients right where they live. You’ll have the tools, autonomy, and administrative support to lead—not just manage.
Oversee nursing, therapy, and aide teams to ensure exceptional patient outcomes
Review OASIS documentation, QAPI data, and audits to keep the branch survey-ready
Mentor and develop field clinicians through coaching and hands-on leadership
Collaborate closely with the Executive Director on outcomes, rehospitalizations, and staffing metrics
Participate in interdisciplinary meetings (IDG, QAPI) and lead continuous improvement efforts
Step into the field only when needed—your focus is on guiding care, not endless visits
Active Illinois RN license
2+ years of home health
1+ year of supervisory or charge nurse experience
Strong knowledge of Medicare guidelines, OASIS, and CHAP or equivalent accreditation standards
Collaborative leadership style with the ability to motivate and grow a team
Pay: $95,000–$113,000 base salary

Based in Joliet with minimal local travel
