What You’ll Do:

• Assess nursing home residents.

• Build care plans. Work with facility teams.

• Track patient progress. Adjust care when needed.

• Keep clean, accurate records.

• Help improve clinical quality.

What You’ll Need:

• Active NP license and board-certified in Texas.

• NPI and DEA licenses.

• Active Medicare provider number (Texas).

• At least 6 months of experience as an NP.

• Preferably 6+ months in SNF or LTC settings.

• Valid TX driver’s license + insurance.
