Here’s what you’ll do:

Your Mission:

  • Lead a care model that’s transforming the way nursing home residents receive care.

  • You’ll manage your own patient panel, handle advanced care planning, conduct thorough assessments, and coordinate with a passionate care team to deliver outstanding outcomes.
  • Help streamline clinical processes like medication reviews and prior authorizations, while ensuring quality care metrics are met (think HEDIS and STAR quality measures).

What You Bring:

  • Unrestricted Advanced Practice Nurse license and national board certification.
  • Eligibility for Medicare participation.
  • 6+ months of experience as an Advanced Practice Provider (geriatric, hospice, or palliative care experience preferred).
  • Strong clinical skills and experience with chronically ill patients, ideally in a nursing home setting.
  • Tech-savvy with excellent communication skills.
