Here’s What You’ll Actually Do:

Manage your own panel of patients

Perform in-depth assessments and manage chronic conditions

Close care gaps (HEDIS and STAR measures)

Lead care planning with a full clinical team

Help patients and families feel confident in their care plan
You’ll Need:

Active, unrestricted APN license + board certification

Eligible to treat Medicare patients

6+ months in skilled nursing or long-term care

Comfortable using digital tools and working with a team
