Our client is a well-established leader in hospice, and looking for passionate RN Case Managers to do meaningful work with an amazing team!
What You’ll Do and Achieve
- Deliver Heartfelt Care: Use your skills to provide high-quality, patient-centered care, focusing on the holistic well-being of each individual. You can expect to complete 20-25 visits per week and manage 11-13 cases.
- Be a Guiding Presence for Families: Offer support, guidance, and comfort to families, helping them navigate complex care journeys with reassurance.
- Experience True Independence: Work autonomously while feeling fully supported by a compassionate, well-coordinated team, managing your caseload with both flexibility and responsibility.
- Champion Dignity: Honor the comfort, preferences, and dignity of each patient, ensuring they feel respected and valued at every step.
What You’ll Bring
- 1+ year(s) of Hospice Experience:
- Willing to complete visits in one of the following counties: Tarrant, Parker, Johnson, Denton, or Collin counties.
