Earn between $134K–$164K base, plus a $15K bonus for clinical quality.
What You’ll Do:

Travel to and provide care in multiple facilities across Texas

Manage a personal panel of patients during each assignment

Complete deep, meaningful assessments

Handle chronic disease management + close care gaps (HEDIS, STAR)

Work with a full care team to plan and adjust treatment

Help patients and families understand their path forward
You’ll Need:

Active, unrestricted APN license in Texas + board certified

Eligible to treat Medicare patients
-
6+ months of experience in skilled nursing or long-term care

Strong with digital tools, collaborative by nature

Must be willing and able to travel across Texas weekly

Comfortable being based in different facilities each week
