Our client is looking for an RN Case Manager to provide field-based hospice care in Victoria, Texas. This role is part of a larger, experienced hospice network with a proven ability to scale care teams and grow in new markets. They're looking for someone reliable, compassionate, and ready to make a difference—without being afraid to be hands-on.
Deliver compassionate hospice care in the field, managing a caseload of patients.
Coordinate and communicate with the interdisciplinary care team, patients, and families.
Ensure compliance with hospice standards and maintain thorough, timely documentation.
Be available for on-call shifts (with extra pay) and occasional after-hours visits (also paid per visit).
Play a key part in providing stability and continuity of care to the community.
RN license in Texas (required).
Hospice experience preferred, but not required—training is available for nurses with the right mindset.
Solid clinical skills and comfort with fieldwork.
High level of dedication, compassion, and reliability.
Willingness to take part in on-call rotation and handle patient visits as needed.
$75,000 to $80,000 base salary depending on experience.
Extra compensation for on-call coverage and after-hours visits.
